Complaints regarding suspected public health nuisances can be filed with the McLean County Health Department by calling (309) 888-5482. Complaints can also be filed by completing a Public Health Complaint Report (Download Below) or faxing the form to (309) 888-5506 or mailing it to:

Be advised that your name and contact information are required to guarantee further investigation of the report. The information you provide will be kept confidential. Please note, however, that your identity may be revealed to law enforcement or investigative bodies in the event of a legal investigation. Anonymous complaints will not be accepted.
