Social Service Department
We understand that change can be difficult. Transitioning from living alone or with family to sharing your home with new people can be unsettling. At McLean County Nursing Home we make every effort to make this transition as easy as possible. We have a full-time Social Services department to assist residents and family with the adjustment to the facility. If you or your loved one has any questions, concerns, or comments please feel free to stop by our office.

The Social Service department will assist you with the paperwork prior to or on the day of admission. Below is some information or copies of what we will need at the time of paperwork.

On or before the day of admission the facility will need a copy of your:
Social Security & Medicare Card, Insurance card (Supplemental Insurance & Medicare Part D). The facility will also need any advance directives you have, such as Living Will, Power of Attorney for Health and/or Finance, or Guardianship.

All personal items are brought to the facility at your own risk. We will mark all personal clothing and items for identification purposes. We ask that you do not bring any valuables, expensive jewelry, medications or electrical appliances.

Below is a suggested list of clothing to bring:

Female: Dresses/pantsuits (5 or 6), Sweater (2), Underwear (10 pairs), Socks/hose (7 pairs), Non-skid slippers (2 pairs), Shoes (2 pairs), Nightgowns/pajamas (5 pairs), Robe (2), Jacket/coat, Electric razor

Male: Pants ( 5 or 6), Shirts ( 5 or 6), Underwear (10 pairs), Socks (7 pairs), Non-skid slippers (2 pairs), Shoes (2 pairs), Pajamas (5), Robe (2), Jacket/coat, sweater, Electric razor
We encourage you to bring in furniture or pictures so the residents can feel comfortable in their new home. If you wish to bring furniture, please see Social Service. The facility prefers washable vinyl lounge chairs. If a cloth chair is brought into the facility and develops an odor, you are responsible for removing the chair from the facility or having it professionally cleaned. The Maintenance department will be happy to assist you with hanging pictures.

If you would like to have snacks in the room, please place them in a sealed container. Any food items that requires refrigeration must be consumed the day it is brought into the facility. There is not adequate refrigerator space to keep large quantities of food. 

A limited number of phone jacks are available. If you are interested and there aren't any phone jacks available your name will be placed on the waiting list. The cost is $1.00 a day. 

McLean County Nursing Home is not responsible for damaged, lost, or misplaced items.  

McLean County Nursing Home has a van for transporting residents to doctor appointments and personal outings. This service will be available to all residents for a nominal fee on a first come, first serve basis. 

The facility van can be used upon request. The following are the rates for these services: 
1. $10.00 for round trip service to doctor's appointments or personal outings within Bloomington/Normal. 
2. $25.00 for round trip service within a 60 mile radius of Bloomington/Normal (Peoria, Champaign, etc.)
3. $15.00 additional fee if the family request, or resident requires a staff escort to a doctor's appointment. 
4. Personal outings with a resident for a staff escort will be determined on an individual basis with consideration given to distance, time requirements, and availability, etc.

Discharge To Home
When you are close to coming off of therapy, our Social Services department facilitates discharges to home. Social Service will set up home health care, outpatient therapy, and/or  adaptive equipment they may need.  

Our goal is for each of our residents to have a successful outcome when they are discharged home.
