When submitting a vital record request, please be sure to submit a copy of the front and back of your state-issued ID or driver’s license, along with your signature.
Notes pertaining to Illinois E-Pay requests
If the mail-to address for your document is different than that associated with the credit card you are using, please note the mail-to address on your ID copy.
If you are ordering more than one record at a time (i.e. a birth and a marriage certificate) to be sent Priority or Express to the same address, you need only pay once for the additional mailing cost. Multiple records will be sent in a single envelope.
Selecting e-check as a payment option will add 3-5 days to the processing time. Therefore, paying by e-check and selecting Priority or Express mail is not advised.
In accordance with Illinois statute, only persons whose names are on a birth record may obtain a copy of the record (i.e., child, mother, and father) unless you have notarized documentation from either parent or the adult child giving you permission to obtain the record. Without notarized documentation you will be denied the record.
