Important Notes: When submitting a vital record request please be sure to submit a copy of the front and back of your state-issued ID or driver’s license along with your signature.
Restrictions
In accordance with Illinois statute, only persons whose names are on a birth record may obtain a copy of the record (i.e., child, mother, and father) unless you have notarized documentation from either parent or the adult child giving you permission to obtain the record. Without notarized documentation you will be denied the record.
