I Would like to File a Public Health Complaint
Do you suspect that unsafe or unsanitary food or water conditions made somebody sick? Or have you noticed a public health nuisance or people smoking in a smoke-free area? Submit a complaint to the McLean County Health Department using the links below.
Foodborne or Waterborne Illness
Complaints regarding suspected foodborne or waterborne illness can be submitted to the McLean County Health Department with using the online Foodborne and Waterborne Illness Investigation Questionnaire. Please keep in mind that the online form requires a 72-hour food history.

File a Public Health Complaint
The Environmental Health Division investigates public health complaints regarding:
- Garbage
- Sewage
- Rubbish
- Animal waste
- Construction or demolition debris
- Landscape waste
- Noise and air pollution
- Open burning
- Weeds
- Insects
- Rodents
- Birds
- Other pests
Complaints regarding suspected public health nuisances can be filed with the McLean County Health Department by submitting a completed Public Health Complaint Report form to the Environmental Health Division by email, fax at (309) 888-5506, or by mail at:

Smoke-Free Illinois Violation
Complaints regarding suspected Smoke-Free Illinois violations are handled by the Health Promotion Division of the McLean County Health Department. Find information about the Smoke-Free Illinois Act and enforcement here .
